    Abstract

    Summary. The aim of the study is to identify risk factors in patients with CVD and people with high risk of CVD. There will be described the management such risks through lifestyle changes and use of medicine, with an objective assessment of the clinical implementation of scientific approaches reflected in the current European Recommendations. Material and methods: 400 coronary patients will be surveyed at the hospital stage and 200 patients with high CVD risk at the outpatient stage from 18 to 80 years. The result of the study will provide an opportunity to analyze the current situation in the prevention of CVD in the Kyrgyz Republic.

  • PYLOT INVESTIGATION ON NONCONVENTIONAL RISK FACTORS DETECTION IN PATIENTS WITH CORONARY HEART DISEASE

    Abstract

    Summary. Complex detection of nonconventional risk factors (NRF) is carried out in patients with coronary heart disease (НФР). There were investigated 54 patients with CHD, stable angina pectoris entered to the Coronary heart Disease and Atherosclerosis Department of the National Center of Cardiology and Therapy. Core questions for clinical interview of the European Society of Cardiologists (ESC), Questionnaire DS-14 were approved In Kyrgyzstan for the first time. The Hamilton Depression and Anxiety Scales were used for evaluation of the depression and anxiety levels. Evaluation of the socio-economic status was carrying out, traditional risk factors were considering. The most prevalent NRF were low income level (68.5%), hostility (44.4%) and type D personality (40.7%). Core question for clinical interview ESC, questionnaire DS-14, Hamilton scales are recommended to cardiologists and therapeutists for timely NRF detection and correction.

  • MECHANICAL VENTILATION IN PATIENTS OF NEUROLOGICAL PROFILE

    Abstract

    Resume: In the article the analysis of tactics and results of prolonged mechanical ventilation in patients of neurological profile. The study was conducted in National Hospital. Criterion for inclusion was prolonged MV – 48 hours by 84 patients with neurological diseases. Ventilation mode, taking care of the upper airways, dentures upper respiratory tract was conducted in compliance with infection control. Significant difference were found in the structure of the indications for mechanical ventilation and the duration of respiratory support, depending on the neurological nosology and localization of the hearth and some risk factors for development of secondary pneumonia.

    Abstract

    Abstract: The result of applying Dimexidum preparation for decontamination of the treatment 78 patients who suffer from echinococcus have been shown. The Dimexidum preparation were used for decontamination of the patient aged between 18-72, of which 46 (59%) are women, 32 (41%) are men who suffer from liver echinococcus, studies have been investigated. With applying 50% of dimexidum solution, and the results have shown that the death of proto-scoxes occurs in 3-4 minutes. Recently (within 1 year) the disease hasn’t evolved.

  • CHARACTER AND FREQUENCY OF COMPLICATIONS IN EMERGENCY ABDOMINAL SURGERY

    Abstract

    Resume: the purpose – to give the analysis of complications in the emergency abdominal surgery. The frequency and nature of complications in 103 patients operated for acute surgical diseases (cholecystitis, strangulated hernia, acute intestinal obstruction, perforation of gastric and duodenal ulcers). Of 103 patients, 24 had complications (23.3%). More often there were wound complications (in 14 of 24), pleurisy in 4, abscesses in 2, peritonitis in 2 and acute pulmonary heart failure in 2. Died 4 (3.9%) of multiple organ failure

  • EVALUATION OF EFFICIENCY OF VARIOUS METHODS OF OPERATIVE INTERVENTIONS IN HYPERTENSIVE HEMORRHAGIC STROKE

    Abstract

    Summary: The results of surgical treatment of patients with hypertensive hemorrhagic stroke showed that an effective surgical method of choice for cerebellar hemispheric localization of stroke hematomas can be recommended - decompression trepanation of the PCF. Resection trepanation of the skull is more promising in patients with subcortical localization with a volume of less than 60 cm3 of hematoma. Bone-plastic trepanation of the skull with removal of intracerebral hematomas is more effective in medial (basal nuclei), ventricular localization with a hematoma volume of more than 60 cm3. Fresotomy with puncture aspiration of the intracerebral hematoma is effective on the 20th day after hemorrhage, regardless of localization and volume of hematoma.

  • NEW APPROACHES TO PREVENTION OF POSTOPERATIVE PURULENT COMPLICATIONS IN ABDOMINAL SURGERY

    Abstract

    Summary: The aim of the study was to evaluate the effectiveness of the use of a solution of decasane and dimexide in the prevention of purulent-inflammatory complications in emergency abdominal surgery. Results are given in 130 patients (control group of 30 patients, application of decasane 50 patients, dimexide administration of 50 patients). The evaluation used clinical, laboratory and instrumental methods of investigation (wound echomorometry). Complications in the control group are 26.7%, with decasane 10% and dimexide 6.0%.

  • THE RESULTS OF LOWER EXTREMITY FROSTBITE SURGICAL TREATMENT

    Abstract

    Summary: In this article we present the evaluation of conservative and surgical treatment of suffering from lower extremity frostbite patients. 67 patients suffering from lower extremity frostbite received treatment in National center of surgery from January 2016 to January 2018. We got 23 patients in 2016, 26 people in 2017 and due to abnormally cold winter we received 18 patients in January 2018. The group of patients was between the ages of 22 and 60. We evaluated their clinical symptoms and "status localis", in addition, all the patients got their complete blood count, biochemical blood test and urine tests. The choice of curing method depended on frostbite degree.

  • RESULTS OF CLINICAL INSTRUMENTAL DIAGNOSIS IN TRAUMATIC BRAIN COMPRE

    Abstract

    Annotation: It was analyzed clinical data and results of management in 104 series with acute stage of Skull Brain injury. Patients admitted with deep disturbances of consciousness: in spoor 17 patients (7-9 scores according to the Glasgow coma scale), in coma of 1-2nd grade 67 patients and in coma of 2-3rd grade 42 patients (305 scores according to the Glasgow coma scale). The Brain contusion of different location was diagnosed in all patients: single – 68, multiple – 37, in combination with epidural and subdural haematomas – 93, combined with intracerebral and ventricular hemorrhages – 34, signs of diffuse axonal injury and brain stem contusion – 9 patients.

  • RESULTS OF TRAUMATIC BRAIN COMPRESSION MANAGEMENT

    Abstract

    Annotation: Postoperative period of traumatic brain compression is characterized by phase in its course. Clinical phase in which patients undergo surgery largely predetermine dynamics. Deepening of decompensation or its reverse development is determined by 4 parameters: general cerebral, vital, local and basing on data from instrumental methods of investigation. The analysis of data in 104 patients with severe traumatic brain injury was given. Age of injured patients ranged from 12 to 79 years. There are 88 males (84.6%) among examined and 16 females (15.4%).

    Abstract

    Summаry: In this state comprehensive assessment of facial pain symptoms in patients with diseases of the nose and paranasal sinuses. The evaluation of pain symptoms in 140 patients with diseases of the nose and paranasal sinuses with the « Pain verbal color test» Revealed that acute rhinosinusitis main component of the pain symptoms is somatic pain. During exacerbation of chronic sinusitis, the main component of pain is a symptom of touch. In patients with chronic rhinitis predominant psychogenic nature of the pain. Evaluation of components that make up the painful symptoms (somatic, sensory and psychogenic), allows you to define the features of the pathogenesis of the disease and determine treatment strategy.

    Summary: The study presents the results of research of auditory and vestibular analyzers in the workers of the “Kumtor” mine functioning in high-elevation conditions. The results obtained indicate a high risk of neurosensory hearing loss in heavy equipment operators with experience of more than five years, caused by the deterioration of the peripheral and central parts of the auditory analyzer.

    Summary: This work represents a post-hoc analysis of labor and delivery records of women with multiple pregnancy, who have been admitted for delivery to the City perinatal center (CPC) from January to December 2017. The survey included analysis of frequency, special aspects of the course of multiple pregnancies and confinement period. The study also examined the literature reports. Indications and contraindications for defining the ideal mode of deliveries through cesarean operation or vaginal delivery have been evaluated.

    Abstract. The aim of investigation was studying of organ saving operations possibilities in ectopic pregnancy. Timely diagnosis and treatment of pregnancy developing outside the cavity of uterus gives a possibility to prevent such dangerous complications as hysterorrexis of fetal poultry, intra – abdominal bleeding, which lead to the loss of female body reproductive function. In this article 3 clinical cases of ectopic pregnancy from different localization are examined. The pregnancies ended with preservation of all organs and full reproductive function.

  • FEATURES OF MEASLES IN KYRGYZSTAN IN 2017-2018

    Abstract

    Summary: An analysis of 162 cases of clinical record of patients with measles was made, who were on inpatient treatment at the Republican Clinical Infectious Diseases Hospital (RCIDH) for the period of 2017-2018. Patients were divided into two groups: 1st group – patients with the diagnosis of measles for 2015, 2nd – with the diagnosis of measles for 2017-2018. Clinical and epidemiological differences between these groups were revealed. The relative risk of developing severe measles in children is presented, who entered in to the RCIDH in 2017-2018.

    Abstract

    Summary: Medical-biological risk factors for the birth of a premature baby were studied. 61 preterm infants were examined. The control group consisted of 30 healthy newborns. The obtained data showed the presence of a number of modifying risk factors for the development of the disease: the older age of mothers (over 35 years), extra- and urogenital diseases of mothers, a burdened obstetric anamnesis, the pathological course of pregnancy.

    Abstract

    Summary: Medical and biological risk factors for the birth of a child with congenital malformations (VLF) are presented. 49 newborns with CDF were analyzed. The control group included 30 healthy newborns. The most significant risk factors for the development of the disease are identified: the older age of mothers (over 35 years), urogenital diseases of mothers, the burden of obstetrical anamnesis, the pathological course of pregnancy.

    Abstract

    Summary: A clinical-laboratory analysis of 52 stories of deceased children from infectious diseases treated in the Republican Clinical Infectious Disease Hospital for the period of 2017 was conducted. In the structure of clinical diagnoses, acute intestinal infections acute respiratory infections and prevailed. The age structure of the deceased patients was dominated by young children (90.3%). Every second child entered the late period of illness in the intensive department, where he was on treatment for 2.9 ± 0.7 days. Analysis of the frequency and duration of emergency syndromes in deceased children showed that the leading and fatal urgent syndrome was cerebral edema.

  • RELATIVE RISK OF DEVELOPING SEVERE FORMS OF DYSENTERY IN INFANTS

    Abstract

    Summary: were observed 384 infants from 0 to 7 years old received to infant infectious disease department in Osh citizien clinical hospital. Sh.Flexneri was leading in etiological structure acute intestinal infections in infants. By using regressive analysis we calculated relative risk of developing severe forms of dysentery based on clinical epidemiological datas. Risk of development severe forms of dysentery is high in infants from 3 months to 1 year and in children 3-6 years old. Probability of severe forms of dysentery increases at late arrival, duration of intoxication symptoms and hemocolitis at refracted premorbid background, development of super infection, complications and emergency syndromes.

  • EFFICACY OF INHALATION WITH ADRENALINE IN ACUTE VIRAL RESPIRATORY INFECTIONS WITH THE DEVELOPMENT OF ACUTE STENOSING LARYNGOTRACHEITIS IN CHILDREN

    Abstract

    Summary: A comparative clinico-laboratory analysis of 64 case histories of children, aged 0 to 6 years, entered the RKIB with ARVI, accompanied by acute stenosing laryngotracheitis (ASLT), which were received in the treatment of inhalation with adrenaline. The patients were divided into 2 groups: Group I - 32 children with LSLT who received inhalation saline-alkaline and hydrocortisone, Group II - 32 patients with LSLT who received inhalations with epinephrine. With inhalation with adrenaline, the positive effect was more rapid than with other types of inhalation. Also, the development of pneumonia in children of Group I was less frequent, and the length of stay in the hospital was reduced.
