  • FEATURES OF CLINICAL STATUS AND INDICATORS CARBOHYDRATE EXCHANGES IN PATIENTS WITH TYPE 2 DIABETES DIABETES IN AMBULATOR CONDITIONS OF ALMATY CITY

    Abstract

    Summary. DM 2 is often combined with cardio vascular diseases (CVD) that promotes progressing of complications. Maintaining such category of patients considerably becomes complicated in out-patient conditions for various reasons. Purpose: to study features of clinical laboratory indicators at patients with a in out-patient conditions of Almaty depending on existence of the accompanying CVD and duration of a disease of DM type 2. All patients with the DM 2 examined by us in out-patient conditions of the city of Almaty have the accompanying cardiac vascular diseases, from them the Arterial hypertension and coronary heart disease are in the lead. With increase in duration of DM 2, the number of the patients having at the same time and cardio- vascular diseases also increases. Average glycohemoglobin index was higher than target. A DM 2, being a self contained factor of high and very high risks warmly vascular disease, will strengthen the influence on progressing of the available cardiovascular complications in out-patient conditions.

  • METHOD FOR PREVENTION OF INSOLVENCY OF INTESTINAL ANASTOMOSES

    Abstract

    Resume. The results of surgical treatment of 7 patients with intestinal anastomosis were analyzed. When performing operations the anastomosis was applied manually using a continuous single-core intestinal suture without capturing the mucous. For better sealing of the suture line the fibrin-collagen substance was used. During the operation there were no complications. In the early postoperative period one patient had an intestinal paresis, which was resolved on forth day, the other deviations were not observed. No patient in the early postoperative period showed signs of insolvency of intestinal anastomoses. No other complications such as anastomotic stenosis were detected during colonoscopy for up to 6 months.

    Abstract

    Resume. Objective is to study indicators of systemic inflammatory reaction at the pa­tients with destructive forms of acute appendicitis complicated by localized peritonitis. The Article provides analysis of indicators at 88 patients with acute appendicitis complicated by localized peritonitis. Clinical and laboratory values are used in the Article. Reasoning of antibiotic and lympho-stimulating therapy for correction of systemic inflammatory response syndrome is provided.

  • DIAGNOSTICS AND SURGICAL TREATMENT OF NON-TUMOR PATHOLOGY OF THE CARDIOESOPHAGEAL ZONE

    Abstract

    Resume. This paper presents the results of surgical treatment of 40 patients with non­neoplastic pathology of the cardio-esophageal zone. After routine clinical and instrumental and specific (endoscopic and x-ray) examination methods, an operation was performed in the volume of krororafiya and fundoplication according to Nissen under endotracheal anes­thesia. There were no specific complications associated with the operation. Lethal cases are not observed. Patients after surgery were discharged from the clinic with improvement. The obtained good results, improving the quality of life of patients justifies our chosen method of surgical treatment of non-tumor pathology of the cardio-esophageal zone.

  • PRESAKRAL TERATOMA WITH SUPPURATION: Rare clinical case

    Abstract

    Resume. Summary: The article presents a clinical observation of retroperitoneal mature teratoma with suppuration in a 30-year-old patient with a rare variant of tumor localization (type 4). Initially, the patient was operated thrice with suspected cystic formation of pelvic region. An magnetic resonance tomography revealed a cystic formation that completely oc­cupied the pelvic floor, probably benign, displacing the uterus and appendages upward, and the rectum anteriorly. The identified presumably benign tumor was completely removed by less traumatic laparotomic access. The postoperative period was uneventful, and was dis­charged on the 9th day in a satisfactory condition. The tumor was a cystic mass consisting of one large cyst, with a thick creamy, dirty yellow color with purulent contents in a volume of 500 ml. at present, there is no pathology with ultrasound, the uterus and rectum are locat­ ed in typical places, there is no dysfunction of the pelvic organs.

  • TO THE ISSUE OF THE PLACE OF COLOSTOMY IN COLON SURGERY

    Abstract

    A retrospective analysis of records of all patients (224 patients), who underwent colosto­ my from 2010 to 2018 at National Surgical Center in Bishkek, Kyrgyzstan, was conducted in November 2018. In general, emergency surgical conditions make the majority of the rea­son for colostomy, 199 (88.8%). The commonest type of colostomy done was Hartman'send colostomy, 183 (81.7%). Loop colostomy made up 36 (16.1%) of all the colostomies. Double barrel and spectacle colostomies together accounted for only 5 (2.2%). Complica­tions were seen on 108 (48.2%) patients. The most common complications were surgical site infection, 51 (23.3%), hospital acquired pneumonia, 23 (10.5%), wound dehiscence, 17 (7.8%), adhesion obstruction, 15 (6.8%) and colostomy retractions, 13 (5.9%). Mortality rate 8.5%.

  • PROGNOSING OF WOUND COMPLICATIONS IN DESTRUCTIVE FORMS OF CHOLECUSTITIS AND MEASURES FOR THEIR PREVENTION

    Abstract

    Summary. The goal is to improve the results of surgical treatment of patients with de­structive forms of cholecystitis by predicting wound complications and their prevention measures. For prediction, the following indicators were used: leukocyte intoxication index (LII), C-reactive protein and echo morphology of wounds. The use of indicators for predict­ing wound complications and the use of preventive measures, taking into account the pre­diction, made it possible to reduce wound complications from 28.5 to 4.0%.

    We conducted prospective trial to study risk factors for unfavorable outcome in patient groups with head injury. We estimated the influence of following risk factors in both groups: trauma mechanism, presence of combined extracranial damages, arterial hypoten­sion, depression of conscious level before operation, data of brain computer tomography (CT), intracranial pressure level and development of complications in postoperative period.

    Abstract

    Summary. The present article shows a clinical case of the course of pregnancy and childbirth in patient N. in the period of 28 weeks, delivered by cesarean section for prema­ ture progressive detachment of a normally located placenta. The clinical picture of the indications for the operative mode of delivery, the features of the course of the cesarean section, as a result of which a bilateral dermoid ovarian cyst was diagnosed, are described.

    Abstract

    Abstract. A retrospective analysis of the outcome of pregnancy and childbirth in 200 women was performed in case of scarring on the uterus after undergoing a cesarean section for 2018 year in the city perinatal center (CPC) of Bishkek. Revealed a significant relation­ ship between the intergestational interval and the thickness of the uterine scar. It was also revealed that the number of cesarean sections surgeries had a significant effect on the thick­ ness and viability of the uterine scar.

  • UNDIAGNOSED CAUSE OF DEATH OF A CHILD. FORENSIC MEDICAL EVALUATION (case from expert practice)

    Abstract

    Summary. Congenital liver pathology - partial atresia of the bile-excreting intrahepatic ducts is a disease with high mortality of up to 90% and is very difficult clinically and mor­ phologically diagnosed. Biliary atresia is characterized by two processes: the complete ab­ sence of the biliary tract and or sluggish inflammatory process. On the case of expert prac­tice, the difficulty of an in vivo clinical diagnosis of this pathology and the establishment of a diagnosis based on histological examination of pieces of the liver has been proved. In this case, the congenital pathology of the liver, partial atresia of the intrahepatic bile ducts led almost to a sudden death of a 2-year-old child.

    Abstract

    Summary. The decline in birth rates and increased life expectancy on a global scale hasled to an increase in the proportion of people over the age of 60 years. This situation is ob­served in our country, as evidenced by the presence of 8 rest houses, in which more than 700 people of elderly and old age live. The number of elderly and old age under the guardi­ anship or supervision of homecare is not known. With inadequate care and harsh treatment of them, various injuries occur in the form of alimentary dystrophy, cachexia, and bedsores, which can be the direct cause of death of such persons. The fact of evidence of such changes in the body of elderly and old aged persons belongs to the forensic experts, and law enforc­ers must give a legal assessment of the actions. It is recommended that the legal form of le­gal responsibility for the inadequate care and harsh treatment of the elderly and old age people.
