  • LESSONS, LEARNED FROM TREATMENT OF CHRONICAL DISEASES IN KYRGYZSTAN: CONSEQUENCES FOR MEDICAL EDUCATION

    Abstract

    Resume: Chronic Noncommunicable Diseases (NCDs) are the leading cause o f morbidity and mortality worldwide, includingKyrgyzstan. As data showed, the management of Chronic NCDs is one of the largest challenges that health systems throughoutthe world currently face and that each system needs to find a locally adapted solution. In this publication the previous studies in Kyrgyzstan focusing on diabetes using a Rapid Assessment tool are presented. The received results highlighted that there are several elements that are necessary for the proper management of NCDs. Some of these elements are clear weaknesses o f the health care provided to patients with NCDs in Kyrgyzstan, namely “Patient education and empowerment” and “Adherence”.
    The conclusion was made, that with the growing burden of Chronic Conditions, medical education in Kyrgyzstan is in need of optimization. The variety o f aspects in health training in Kyrgyzstan can be improved. These range from clinical skills to softer skills and even Public Health training. Changes in training will impact how the health system is organised.

    Abstract

    Resume: The investigation tasks were to observe congenital malformation prevalence in the regions o f Ukraine, to indicate their risk factors and to estimate the efficacy o f their primary and secondary prophylaxis.
    According to the Ministry of Healthcare data the frequency of congenital malformations was estimated among born alive, stillborne and genetically caused abortions during 2002-2011. Case-control study was performed.
    Congenital malformations prevalence among born alive, still-borne and genetically caused abortions was (26,83±0,08) %o during 2002-2011, it was fluctuated in different regions. Congenital malformations priority risk factors were underlined; they were mother’s chronicle infectious and non-infectious diseases (including endocrine diseases), smoking, professional hazards and age.
    The efficacy of congenital malformations primary and secondary prophylaxis was shown.

    Abstract

    Resume. For thyroid hypofunction studying and coronary angiography results in patients with coronary heart disease we conducted a retrospective study. We analyzed patients histories. In all patients thyroid stimulating hormone examination and coronary angiography was provided. We defined a correlation between the worse results o f coronary angiography and low function of thyroid gland. Hence, these is an opportunity o f participation of hypothyroidism in atherogenesis development.

    Abstract

    In randomize parallel controllable clinical trial the experimental substantiation of application o f multicomponent ointment Levomekol for intraoperative preventive maintenance of adhesive process o f abdomen cavity is given. For experimental research we use the 60 rats o f “Vistar” line. In control groups we used a mixture o f Zhenchevsky and physiological solution NaCl. High efficiency o f ointment of Levomekol in intraoperative preventive maintenance of adhesive process o f abdomen of cavity is proved.

  • THE ASSESSMENT OF LIFE QUALITY OF PATIENTS WITH THE DEFECT OF MIDDLE ZONE OF THE FACE

    Abstract

    Resume: 84 patients were examined and treated at stomatological clinic of Kaz A.M.U named after S.D Asfendiakov. The purpose of this study was the assessment of life quality o f patients with the defect of middle zone o f the face before rendering orthopedic stomatological aid. The patients were divided into 3 clinical groups depending on the character of the defect of maxillo-facial region. The study used functional tests and cards o f disturbances, determined the rate o f functional, anatomical disturbances and showed that life quality was considerably impaired. Deformation of facial skeleton, disfiguration of face and disturbances o f vitally important functions negatively influences on human psychic state and leads to social desadaptation. Cards of examination and functional tests may be used for the assessment of life quality of patients with maxillo-facial defect before and after orthopedic treatment.

    Abstract

    Summary:The results of surgical treatment of chronic osteomyelitis in 106 patients with diabetes mellitus were analyzed. A complex method of therapy o f chronic osteomyelitis in patients with diabetes melittusproved to be more effective in comparison with the traditional one, in 2,5 times the relapses and average treatment terms.

  • STATE OF THE CYTOKINE BALANCE IN TOXOCARIASIS

    Abstract

    The paper shows that in patients with toxocariasis during the detailed clinical manifestations of an imbalance is detected with proinflammatory cytokines (TNF-a and IL-6) and anti-inflammatory (IL-4) functions, which can contribute to the immunoinflammatory reactions and cause severe disease.

    Abstract

    Some epidemiological aspects o f tick-born encephalitis (TBE) in Kyrgyz Republic are discussed in given paper. Morbidity of tick-born encephalitis is remaining stable in the condition o f presence o f the natural foci of the infection. The annual dynamics is characterized by spring-summer seasonality.
    The clinical manifestation o f the disease in modern time is characterizing by worsening o f condition o f patients because of nervous forms prevalence. The active people who are able to work are in risk group. To prevent clinical cases and new cases of TBE, education of population is necessary.

  • PECULIARITIES OF DIAGNOSTICS AND TREATMENT OF PATIENTS WITH CANINES AND INCISORS RETENTION OF MAXILLA

    Abstract

    The study of peculiarities o f diagnostics and treatment o f patients with canines and incisors retention o f maxilla, done. The sample size was 24 patient o f 14-24 years old. Also, 45 X-rays of patients were analyzed. This patients were admitted with complains o f not eruption of constant teeth of maxilla. The X-ray permits to detect only mesiodistal slope o f canine. The periapical sighting shots permit to receive the most important information about impacted teeth. The Clarks method permits the most precisely to detect the canine location. Hence, the orthodontist, who is sending the patient to dentist surgeon, can demonstrate the location for surgical denudation o f impacted canine from the side, where it presents.
    The prognosis of orthodontic treatment o f impacted teeth with expressed periodontal fissure is good. In the complex treatment the most difficulty is the fixation o f ligature on impacted teeth. The usage o f metallic cap with hooks or loop, which is fixed on the root o f impacted tooth, gives the best results.

  • RISK FACTORS OF THE NON-INFECTIOUS DISEASES AMONG MEDICAL STUDENTS

    Abstract

    The aim of our research was to study the prevalence of main risk factors o f non-infectious diseases among medical students in Kyrgyzstan.The participants of the study were 643 students of 1-6 courses of Kyrgyz State Medical Academy, including 234 boys and 409 girls, their middle age was 20,1 ± 2,2 years. Students had to fill out special «health improvement card» to evaluate life style/behavioral risk factors. Biological risk factors were examined by measuring blood pressure, body mass index, glucose and cholesterol. Among the students relatively high frequency o f various risk factors o f non-infectious diseases, including high cholesterol (11.8%) and sugar (3.2%) o f blood, smoking (10%), overweight (7.4%), physical inactivity ( 6.4%), increased systolic (4.2%) and diastolic (2.8%) blood pressure were identified. The high frequency o f risk factors o f non-infectious diseases requires to improve the teaching o f medical students issues the prophylactic method o f prevention of non-infectious diseases and improving their competence in the area o f formation healthy life styles.

    Authors investigated 102 patients in age from 16 to 65 years with complicated thoracolumbar spine injuries. Optimal surgical treatment method for complicated spine injuries using close vertebrae reclination with a dynamic control was elaborated. Basing on the received results the differential diagnostic signs and efficacy o f surgical methods have been determined.

    Neurogenic bladder is dominated (60%) among functional disorder of urinary tract. Main symptoms o f neurogenic bladder are frequent urination (55,3%) and night urinary incontinence (43%). Majority acute and chronic inflammatory diseases of kidneys are appeared against the background of neurogenic hyperactive urinary bladder in children 3-7 years old (p< 0,05).

    It was established that most frequently used schemes of antibacterials in postpartal endometritis are inadequate to clinical protocol recommendations on the treatment of postpartal endometritis approved by Ministry of Health of Kyrgyz Republic. The use of cefasoline in patients with postpartal endometritis after operative labor is also irrational because this drug was used for surgical prophylaxis.

  • REPRODUCTIVE HEALTH OF WOMEN AFTER MYOMECTOMY

    Abstract

    The paper presents the results of treatment of 195 patients with myo-my uterus. The dependence of the duration of the operation of myomectomy, blood volume, frequency of relapses on the number and location o f fibroids removed. Among patients who have lost a pregnancy, were twice as likely (8 of 16) than takeaways (24 o f 94) met women who. surgery was performed with the opening of the uterus (50% and 25.5%, respectively).

  • MANIFESTATIONS OF UTERINE LEIOMYOMA IN POSTMENOPAUSAL

    Abstract

    The paper presents the results o f a study o f flow - Postma nopauzalnogo period in patients with uterine leiomyomas (LM ). Spotted this dependence that patients whose postmenopausal uterine regression did not occur, later had an average age of menopause. Patients without recourse LM postmenopausal significantly more likely to suffer from gynecological diseases such as endometriosis (25.0 %) and endometrial hyperplasia (half of patients).

  • INITIAL LEVEL OF THE STUDENT’S KNOWLEDGE AS A PASSING ASSESSMENT OF THEIR SUBJECT LEARNING QUALITY

    Abstract

    In the system o f quality assessment o f learning o f the information block by students it is necessaiy to measure the level of knowledge. The purpose of the study was to analyze the level of knowledge of the 2nd year students o f KSMA on specific fundamental subjects. 289 students of the 2nd year o f the medical faculty completed questionnaires. Questionnaire included questions on subjects o f normal anatomy and normal physiology, rights o f students to resit an examination, and the ability of communication with patients. Results o f survey revealed that only 11% o f the 2011-2012 school year students were able to communicate with patients, 18.3% of students o f the 2010-2011 school year still do not know the number of anatomical heart cavities, 15% o f students were not informed about the upcoming exams for the second course, up to 77% were not informed about the form of exams, 94.6% of the students were not aware about a limit to reset an examination. Thus, the analysis of the initial level of knowledge of 2nd year students showed lack of knowledge of students on normal anatomy and physiology, communication with patients, some o f the students are not well informed about the upcoming school subjects. Revealed results clearly reflected in the learning process and the quality o f clinical disciplines in subsequent courses.
