    Abstract

    The article presents an analysis of the results of treatment of patients who underwent repeated surgical interventions for the development of peritonitis in the early postoperative period, and in a set of traditional methods of detoxification in some cases hemosorption (HS) was used in combination with ultraviolet radiation of autoblood (UVI), and also hyperbaric oxygen (HBO) as a means of treating an infectious process of the abdominal cavity, accompanied by severe endotoxicosis. It is shown that elimination of toxins from the body, improvement of the microcirculatory bed with a corresponding restoration of trophism of tissues, caused by the impact of these methods of intensive therapy, may be more effective means in eliminating the consequences of septic complications from the abdominal cavity organs.

  • SURVIVAL OF PATIENTS WITH EDUCATION OF ETHESIS AFTER DIFFERENT MODIFICATIONS OF RADICAL OPERATIONS

    Abstract

    The analysis of the survival rate of patients with cancer of the esophagus of various stages after the radical surgery was carried out. It is noted that the average five-year survival rate after single-step operations was 10.0%. At the same time, after Dobromyslov-Torek’s operation, no one patient survived the 5-year milestone. In Lewis operation this index was 8.0%, in Garlock’s operations - 11.8%. The adjusted five-year survival rate for Lewis was 12.0 ± 8.6%, for Garlock’s - 12.4 ± 8.1%. When calculating the survival rate by the actual method, where all non tracked cases were taken into account, the reasons for retirement, the causes of mortality, the five-year survival rate after the Lewis operation was 25.0%, after Garlock’s operation - 17.6%, in general for all operations for the entire period - 20.0% We consider the calculation of the survival rate of patients with the topical method is more objective. The prognosis of the operated patients with cancer of the thoracic esophagus is more affected by the length of the tumor in the esophagus, its morphological structure and the presence of metastases in the regional lymph nodes.

    Abstract

    The article presents the results of treatment of 39 patients with nonspecific empyema of the pleura who were hospitalized at the National Surgical Center of the Ministry of Health of the Kyrgyz Republic. The analysis of the causes of EP, the features of the clinical picture and treatment options were determined. Complications were observed in 7 patients, 5 (12.8%) patients died.

  • OMENOBURSOTOMY AS A METHOD OF SURGICAL TREATMENT OF DESTRUCTIVE FORMS OF ACUTE PANCREATITIS

    Abstract

    Of the 2166 admitted patients with a diagnosis of Acute Pancreatitis, 156 were operated on, and the remaining patients, after the conservative measures taken, were discharged with improvement. During the operation, adipose pancreatic necrosis was detected in 16 (10.2%) cases, mixed in 52 (33.6%) cases, hemorrhagic pancreatic necrosis in 82 (52.5%) cases and purulent pancreatitis. Performed an original technique of surgical intervention, developed at the National surgical center omentobursostoma (OBS) with parietal deperitonizatsiey and the mobilization of the pancreas from the retroperitoneal space, drainage retro-pancreatic tissue and biliary tract, total extended retroperitoneal neuro-vegetative blockade, local hypothermia, followed by regularly scheduled sequestrectomy and sanitation cavity stuffing bags.

  • THE ANALYSIS OF RESULTS OF DIAGNOSTICS AND TACTICS OF TREATMENT OF PATIENTS WITH ADHESIVE INTESTINAL OBSTRUCTION

    Abstract

    The analysis of results of diagnostics and tactics of treatment of 157 patients arrived with various forms of adhesive intestinal obstruction (AIO) in NSC MH KR for 2008 is put in a work basis. 99,6 % of patients earlier have operations on an abdominal cavity from 1 to 6 times. 28 % are repeatedly operated concerning acute AIO. From 157 patients 88 patients with symptoms compensated AIO are carried out conservative medical actions. 69 patients (36 women, 33 men) are operated. During operative intervention found out various variants of unions of an abdominal cavity. Proceeding from it definition of surgical tactics in each specific case dares individually, considering character and prevalence of adhesive process, level of intestinal impassability, degree of decompensate of intestine, presence or absence of a peritonitis, a condition of the patient and qualification of the surgeon.

    Abstract

    In this report, we present the results of the use of intra-extracystic sanation in complicated cholecystitis, the state of hemostasis and gallbladder microflora. Cholecystectomy under video laparoscopic control with destructive cholecystitis allows to speed up the operation time, reduce the number of conversions, intra-and postoperative complications. After surgery, there is hypercoagulation and depression of fibrinolysis, a decrease in the adhesive and aggregation activity of the platelets, which must be considered when maintaining the postoperative period. The microflora of the gallbladder in many patients is resistant to most antibiotics.

    Abstract

    The analysis of 1332 laparoscopic cholecystectomy. In the presence of obstructive cholecystitis, it is necessary preliminarily make a percutaneous puncture of the gallbladder, evacuate the contents and wash the cavity of the gallbladder with an antiseptic solution clean.
    Complications required laparotomy in 5 patients, relaparoscopy in 1 patient and puncture under ultrasound control in 6 patients.

    Abstract

    In order to analyze the features of surgical tactics in acute suppurative inflammatory diseases of the pelvic organs using laparoscopic techniques, we analyzed 65 clinical cases of purulent inflammatory formations of the pelvic organs. The age of patients ranged from 17 to 45 years, averaging 36.4 years. From the analysis of our data, it is clear that piosalpinxes (45.3%) prevailed in patients requiring surgical treatment, one and two-sided tubo- ovarian formations (23%) ranked second, and one-way and two-sided pyovars (21.7%).

  • ABOUT DIFFERENTIAL DIAGNOSTIC DIFFICULTIES IN ACUTE APPENDICITY IN WOMEN

    Abstract

    Purpose: comparative assessment of clinical and laparoscopic data of acute appendicitis and acute inflammatory diseases of uterine appendages.
    Materials and methods. A total of 335 patients who were treated in the NSC MH KR were examined. Age ranged from 17 to 42 years. The main group of the examined patients consisted of 101 patients with acute appendicitis and 65 with acute inflammation of the internal genital organs, who underwent laparoscopic examination for the purpose of differential diagnosis.
    Of the 101 patients who underwent laparoscopy, 22 had acute catarrhal appendicitis, and 79 had complicated forms of acute appendicitis with peritoneal events. All patients diagnosed with acute appendicitis were operated on. Laparoscopic or traditional appendectomy was performed. There were no complications.

    Abstract

    An analysis of the treatment of patients with acute intestinal obstruction of various etiologies who were treated at the NHC clinic of the Ministry of Health of the Kyrgyz Republic is presented. The use of the diagnostic, tactical and treatment provisions proposed by the authors allowed the mortality rate to be stabilized among patients with acute small bowel obstruction at the level of 8.1%.
