ТУБЕРКУЛЕЗ ЖЕНСКИХ ПОЛОВЫХ ОРГАНОВ (ОБЗОР ЛИТЕРАТУРЫ)
Аннотация
Туберкулез женских половых органов (ТЖПО) вызывается бациллами Mycobacteria tuberculosis и обычно является вторичным по отношению к туберкулезу легких. Распространенность туберкулеза женских половых органов не точная, так как в большинстве случаев вовремя не диагностируется. В разных странах заболеваемость ТЖПО варьируется от 1% до 30%. Во время первичного инфицирования M. tuberculosis, распространение происходит гематогенным и/или лимфогенным путями и оседают в различных органах в виде латентных форм. У 5-15% инфицированных пациентов латентные формы микобактерий, под воздействием различных факторов реактивируются, вызывая вторичное заболевание. Туберкулез женских половых органов, особенно в слаборазвитых странах, продолжает оставаться основной причиной нарушения менструального цикла и бесплодия. Наиболее распространенным проявлением ТЖПО является бесплодие из-за поврежденных фаллопиевых труб, очень низкой восприимчивости эндометрия или повреждения яичников с низким овариальным резервом и объемом. Диагноз ТЖПО требует подтверждения комбинацией тестов. Гинекологи могут начать с получения информации через историю болезни пациента, общий физический осмотр, осмотр брюшной полости и гинекологический осмотр. Рентгенография грудной клетки может показать активные легочные или старые зажившие поражения перенесенного туберкулеза, что приводит к подозрениям на поражение гениталий. Обычные методы диагностики ТЖПО включают различные методы визуализации, бактериологические, молекулярные и патологические методы, которые дают положительный результат только у небольшого процента пациентов, в результате чего во многих случаях состояние остается не выявленным. Как и при туберкулезе легких, лечение ТЖПО длится в общей сложности шесть месяцев. Хирургическое вмешательство не рекомендуется при ТЖПО из-за многочисленных рисков, и рекомендуется пероральная противотуберкулезная терапия. Большое значение имеет информирование больных туберкулезом легких о необходимости соблюдения мер инфекционного, а также о соблюдении режима рекомендованной терапии.
